You may also use use our custom referral form or your own stationery.

If you require urgent appointment for your patients, Please forward a request on your letterhead or call our reception staff.

For Emergency Appointments

Please call our reception team or Dr Sirigiri directly on his mobile. Dr Sirigiri has instructed our reception team to always force-book any direct requests from GPs/ referrers. If you require Dr Sirigiri's mobile number, please forward a request on your practice letterhead, or call our receptionist.

Dr Sirigiri is always available to speak to you personally. Please call Dr Sirigiri directly on his mobile. He will endeavour to answer all calls from GPs/ referrers at the earliest opportunity.
